Just for new patients
For your first visit, plan to arrive at least 15 minutes early to complete initial paperwork, including a medical history form. If you prefer, you may download the forms below, fill them out ahead of time, and bring them with you to your first appointment.
New Patient Forms
All patients should provide us with information regarding current medication, including the drug name and dosage. Please also bring information regarding your insurance coverage. If any of this information changes between visits, let us know so that we may update your records.

About Insurance
Patients must bring their insurance card to their appointment. If you do not provide us with insurance information, you will be considered a "self-pay" patient for the visit and payment will be expected at your appointment.
We currently accept the following insurance plans:
- Blue Cross/Blue Shield
- Cofinity/Aetna
- Great West
- Medicare
- PHCS
- Rocky Mountain HMO
- San Luis Valley HMO
- Tricare
- United Healthcare/Pacificare

Payment Policy
If you do not have insurance, or if you have an insurance plan with which we do not participate, payment in full is expected at the time of your visit. Co-pays will also need to be paid at the time of the visit. As a convenience, we may be able to bill your insurance company for you. However, you remain responsible for payment of the bill regardless of your insurance status. We accept payment in the form of cash, check and credit card.

Cancellation Policy
Our office requires 24 hours notice to cancel an appointment. If less than 1 full working day's notice is provided, a cancellation fee of $50 will be added to your account.
